St. Luke’s Building Usage and Rental Form Information:
Deadline - Please complete, sign & submit the Building Usage & Rental Form two weeks before the building usage/rental
Approval - Please allow two St. Luke’s business days for review/approval. Please be aware that St. Luke’s business days are Tuesdays, Wednesdays and Thursdays.
Agreement - All building use/rental parties agree to supervise participants, clean up, restore the room/s to their pre-event condition, communicate any problems with the building, close all windows/doors; turn off lights and lock-up building after event.
